CARER SUPPORT

Overview 

Carer Support Services is conducted by FamilyCare and operates throughout the West Hume region comprising the City of Greater Shepparton and the Shires of Moira, Strathbogie, Mitchell & Murrindindi.

The service essentially aims to support carers through the provision of short-term respite, information or support.

For the purpose of the program a carer is defined as a person who is unpaid and voluntarily cares for a person who is frail aged or has a disability or a mental illness.

While flexible responses are implemented following individual case assessment, Carer Support Services provided via this program may include:

  • Short term in and out of home respite (not residential);
  • Short term provision of meals, home care or personal care;
  • Provision of information regarding local service availability and specific health, ageing or disability issues;
  • Information and linkages with self-help, community support groups or day programs;
  • Communication equipment for the frail aged.

Referral Process

Carers can self refer to the service via telephone or in person.

Referrals from other health services or professionals are required to be completed using the single page Carer Support Services program referral or the Service Coordination Tool (SCTT).

Eligibility Criteria 

Carers of people aged 65+ (50+ for Aboriginal or Torres Strait Islanders) or those suffering from an intellectual, sensory or debilitating and permanent or ongoing physical disability or mental illness.

Cost of Service 

Services are generally provided free of charge but a co-payment is charged if the carer is employed.
